Why Radiographs are important?

What you as parents see in your child’s mouth is just the tip of the iceberg, and a radiograph helps us give a clear picture with regards to the depth of the cavity, How close it is to the nerve, the position of the developing permanent bud, restorability of the tooth, and a lot more. However, an IOPA/Radiograph is a two-dimensional picture of a three-dimensional structure, hence most of the time whether to be conservative and do an Indirect Pulp Treatment IPT, or go ahead and a pulpotomy treatment or complete the Pulpectomy root canal treatment procedure. Let’s understand one at a time.

So, what do we see here?

  • We see the first primary molar with a restoration/filling between two teeth failing.
  • We see multisurface fillings done in the past that did not pass the test of time and hence in such cases a full-coverage crown was the best treatment forward.
  • We see the second primary molar with deep grooves causing food lodgement, ideally should have been sealant from the oral distress.

What went wrong?
Ideally, let’s not just treat cavities(DRILL n FILL) let’s treat dental disease. This 4-year-old landed up with what we call secondary cavities. The primary cavities were drilled and filled, however, due to a multitude of reasons, i.e improper brushing habits, absence of flossing, between-meal snacking, and not keeping follow-up, lead to additional plaque built up and what we call a new cavity under the existing or dilapidated restoration (secondary cavities).
So how a preformed metal crown would have helped avoid the rigmarole?

A preformed metal crown covers the cavitated milk tooth 360 degrees, avoiding any plaque/biofilm built which if left in-situ would cause secondary cavities. So when you cover the tooth completely, you provide an environment for repair, you cut off the nutritional supply to the bacteria that cause these cavities, and hence prevent the progression of the cavities under the crown.
So what is an indirect pulp therapy IPT?

In indirect therapy, after the removal of old restoration and the secondary cavities, we are close to the nerve/pulp, but the nerve has not opened up i.e you don’t see a red spot. Then the tooth is a good candidate for IPT. In the case discussed above, you see a clean cavity that has been packed with a biomaterial BIODENTINE followed by a stainless steel crown by 3M.

Why do we need Biomaterials?
Your Pulp/Nerve is a living tissue. Now, when we get that close to it, we need materials that would provide an environment conducive to repair. When we pack Restorations/ Fillings so close to the nerve, the tooth will eventually land up with an Abscess or Pus-filled cavity.

When restorative materials are packed close to the nerve, it may irreparably damage the nerve leading to pus formation as seen here, and hence a thorough clinical and radiographic evaluation is a MUST!

So when do we do Root Canals Treatment for Children in Pune?
As far as possible, our efforts are to keep the tooth in its natural state, and IPT is one such procedure providing an environment conducive to repair.
However, while selective removal, if we see a red spot, which implies that the nerve has opened up, we may have to do
A. Pulpotomy procedure: It’s amputating the top portion of the infected nerve and preserves the root portion of it.
In case we are unable to arrest the bleeding from the tooth we may have to do a complete nerve extirpation or Pulpectomy procedure.

What does a pulp exposure look like?

Such extensive fillings should always followed by a crown as discussed above!

Hence, some decisions will be a clinical call, while we work on your child’s tooth!

If bleeding from the tooth is controlled we proceed with Biodentine Pulpotomy.

If uncontrolled bleeding from the tooth, we proceed with Complete root canal treatment, Pulpectomy

Conclusion:

  1. Every tooth does not need a root canal!
  2. IPT has better success rates of 95-98% than invasive procedures!
  3. It’s better to cap/crown extensive cavities, or cavities between two teeth!

Frequently Asked Questions

Root Canal Treatment in Pune (RCT) is most commonly a painless procedure (more than 90% to be precise in Pune) that will preserve your tooth and keep the structure from becoming damaged due to infection or trauma.

A root canal is a treatment used to repair and save a tooth that is badly decayed or infected. A tooth’s nerve and pulp can become irritated, inflamed, and infected due to deep decay; repeated dental procedures on a tooth; or large fillings, a crack, or chip in the tooth.

A root canal is performed when the endodontist removes the infected pulp and nerve in the root of the tooth, cleans and shapes the inside of the root canal, then fills and seals the space. Afterward, your dentist will place a crown on the tooth to protect and restore it to its original function.

Root canal treatment in Shivaji Nagar Pune are performed under local anesthesia and generally no more painful than other routine dental procedures like wisdom teeth removal or fillings. In fact, root canal treatment doesn’t cause pain but actually relieves it. With modern techniques and anesthetics, the vast majority of patients report that they are comfortable during the procedure. Root canal treatment actually relieves this pain! Thanks to technological advances, root canals are about as painful as getting a dental filling, which is not painful at all.

The base cost is Rs. 5,000. The charges vary depending upon

  1. The extent of infection present in the tooth, and
  2. The required treatment. The final estimate will be discussed by our team after we examine your tooth clinically and use a radiograph.
